In-Network Health Insurance

In-network with Kaiser Permanente, Moda Health, PacificSource Health Plans, and OHP Trillium insurance plans.

We can accept your PRIMARY insurance plan with these providers:

  • Kaiser

  • MODA

  • PacificSource (some plans)

  • OHP Trillium

We are happy to check your insurance benefits before your first appointment.

As health insurance companies remind you, coverage quoted is not a guarantee of payment or verifies eligibility. Payment of benefits are subject to all terms, conditions, limitations, and exclusions.You are ultimately responsible for all fees if your insurance does not pay.

What does this mean for you?

  1. You are responsible for verifying your benefits, especially if they do not match the information provided by your insurance in your handbook or member service portal. 

  2. Benefits sent to you by Counseling Services of Portland are quoted by you insurance company.

  3. If your therapist is newly licensed or a registered associate, you will see their billing supervisor’s name on your E.O.B. for your sessions.

  4. Counseling Services of Portland is not responsible for any information that is not quoted correctly or left out by your insurance company. 

  5. If your insurance denies payment for service, you will be responsible for payment.

Private Pay Rates

The rates listed below are for those who are not using their insurance or do not have insurance. Our contracted therapists reserve the ability to reserve fees for their private pay clients in cases of hardship. These adjustments are done on a case-by-case basis.

We do not bill Out-of-Network insurance.

If you are interested paying our cash rates for your out-of-network insurance benefits and need a superbill, we can provide those upon request to our Lead Billing Specialist.

Master’s Level Providers (LPC, LCSW, LMFT)

  • Individual Intake Appointment: $240

  • Individual Follow-up Appointment: $205

  • Family/Couple Intake Appointment: $240

  • Family/Couple Follow-up Appointment: $195

Doctorate Level Providers (PhD & PsyD)

  • Individual Intake Appointment: $320

  • Individual Follow-up Appointment: $270

  • Family/Couple Intake Appointment: $320

  • Family/Couple Follow-up Appointment: $200

How to Pay

You can pay with cash, check, debit card, credit card, or HSA/FSA card.

We are able to accept all major credit and debit cards (Visa, Discover, MasterCard, American Express, JCB, and Union Pay).

The cost of counseling is an investment in your well being. We are committed to doing what we possibly can to make it work for you.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your health care will cost.

Under the law, health care providers need to give patients who don’t have certain types of health care coverage or who are not using certain types of health care coverage an estimate of their bill for health care items and services before those items or services are provided. 

  • You have the right to receive a Good Faith Estimate for the total expected cost of any health care items or services upon request or when scheduling such items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • If you schedule a health care item or service at least 3 business days in advance, make sure your healthcare provider or facility gives you a Good Faith Estimate in writing within 1 business day after scheduling. If you schedule a health care item or service at least 10 business days in advance, make sure your healthcare provider or facility gives you a Good Faith Estimate in writing within 3 business days after scheduling. You can also ask any health care provider or facility for a Good Faith Estimate before you schedule an item or service. If you do, make sure the health care provider or facility gives you a Good Faith Estimate in writing within 3 business days after you ask.

  • If you receive a bill that is at least $400 more for any provider or facility than your Good Faith Estimate from that provider or facility, you can dispute the bill. 

  • Make sure to save a copy or picture of your Good Faith Estimate and the bill.
